Welcome to the Ralph H. Johnson (RHJ) VA Medical Center where we honor America's Veterans by providing exceptional health care that improves their health and well-being. All students, including residents, assigned to the RHJ VAMC will need to complete the mandatory VA requirements and provide documentation in accordance with your trainee level listed below.  All forms must be submitted to Education Service (or to your instructor if assigned) at least two weeks prior to your scheduled clinical training date.

FORM SUBMISSION INSTRUCTIONS

Forward completed forms to the linked e-mail address below which will expedite form processing and orientation.

All forms can be submitted via this address.
(Please bring original signed documents to the Orientation and copies of:
     1. Current PPD (within one year)
     2. Professional Licenses (if applicable)
     3. Current Basic Life Support (BLS) certification
     4. Current Advanced Cardiac Life Support (ACLS) required for licensed trainees rotating in ICU, Emergency Department
     and Interventional Clinical Areas
