DAV Driver Application Process

Please complete the following DAV Driver application packet below.  Once complete, contact Voluntary Service, (843) 789-7488 or (843) 789-7230, to set up an appointment.  Please bring your completed DAV Driver application packet, a copy of your insurance card and your driver's license.  The Voluntary Service office is located on the fifth floor, room B506.

You will then be asked to pass a physical, PPD test, hearing exam and background check.  For the background check, please bring two forms of acceptable ID listed on VA Form 0711 (Applicant Identity Verification section).  The background check process takes approximately two weeks.  When the background check has cleared, a Voluntary Service staff member will contact you and schedule you to attend AARP Safe Driving Course and Charleston VAMC Volunteer Orientation.

Veteran may bring an escort/attendant only if:

• Veteran is awarded Aid and Attendance and that is on record at VA or
• VA physician certifies in writing or email to Harriett.Hudson@va.gov that an accompanying caregiver is medically necessary
