Patient and Family Centered Care (PFCC)

What is Patient and Family Centered Care (PFCC)?

Patient and Family centered care is an approach to the planning, delivery and evaluation of healthcare that is grounded in mutually beneficial partnerships among healthcare providers, patients and families. It redefines the relationships in healthcare.

Principles Driving the PFCC Model

  • People are treated with dignity and respect 
  • Healthcare providers communicate and share complete and unbiased information with patients and families in ways that are affirming and useful
  • Individuals and families build on their strengths through participation in experiences that enhance control and independence
  • Collaboration among patients, families and providers occurs in policy and program development and professional education as well as in the delivery of care

Charleston VA’s goal for PFCC

To collaborate with our patients and their families to provide a warm, comfortable and friendly environment that supports healing either through a practice or experience. We are utilizing the VHA’s 12 Principles of PFCC to help transform healthcare with the feedback from our Veterans.
